**Job summary**

North Staffordshire GP Federation (NSGPF) is delighted to be facilitating and co-ordinating the role of a Primary Care Paramedic for Newcastle Central PCN integral to the NHSE future plan for general practice which encourages the development of this role with the key focus around frailty and care home monitoring.

Newcastle Central Primary Care Network (PCN) has a patient population of approximately 40,000. This role will require the successful applicant to work across the PCN Network.

**Main duties of the job**

The role has a focus on visiting patients in their own homes and in care home settings; taking medical histories, carrying out examinations in particular fraility assessments and falls reviews, updating records, ordering and analysing test results, and using problem solving and clinical decision making skills to establish a working diagnosis. Within this position you will directly improve patient care and support the efficient running of a new model of general practice. You will work with other members of the clinical team within the Practice consisting of GPs, Nurses, Clinical Pharmacists, and Social Prescriber. This role will also involve supporting the MDTs within the Primary Care Network (PCN) to deliver effective, co-ordinated care for vulnerable and frail adults, particularly those at high risk of a hospital emergency admission, emergency department (ED) attendances or out of hours care.

Experience in General Practice would be an advantage but is not essential if you are clinically up-to-date and can show a firm understanding of the role within a Primary Care setting.

Travel and carrying out home visits/care home visits will be necessary, so your own transport is essential.

**Job Overview**

The Paramedic will be a registered paramedic with post graduate qualifications who acts within their professional boundaries. You will work in accordance with the Health & Care Professions Council (HCPC) Standards of conduct, performance and ethics and; Standards of proficiency for Paramedics. You will also take account of the NHS Constitution in your decisions and actions.

This role is expected to develop and evolve over time as the successful post holder gains experience.

You will work closely with the GPs to advise on treatment and healthcare for patients, to include those presenting in the primary care setting and those in the community.

You will triage acute patients presenting in the surgery environment.

You will attend home visits to include residential and nursing homes, where they will liaise with staff, family and other stakeholders in the production of care plans to be shared with other providers

You will work collaboratively with the multi-disciplinary team to meet the needs of our patients.
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
- Assess patients presenting with acute illness or for review of chronic disease conditions.
- Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and follow up as appropriate.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Alongside the GPs, to provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan
