KEY RESPONSIBILITIES Work with individual patients to construct and deliver tailored physical activity programmes. Offer individual consultation sessions focusing on identifying the patients stage of change and their motivation for change. Support patients to adopt health-enhancing behaviours by setting and recording goals to undertake clearly defined behaviours, in particular contexts, over a specified time. Develop a seamless care pathway for patients with musculoskeletal conditions.

Contribute to and assist with ongoing service evaluation and audit of exercise interventions including the preparing of reports. Represent a community rehabilitative approach to address physical activity issues. To work evenings if required. Please see attached JD for further details
