**Job Title**: Homecare Nurse

**Location**: Portsmouth

**Hourly Pay Rate**: £29,000 to £32,000 per year (dependent on experience)

**Hours**: 37.5 hours per week

**Contract Length**: Permanent

Grafton Recruitment are now seeking to appoint a Homecare Nurse to our client, based in Portsmouth. The Homecare Nurse needs to be a highly motivated and enthusiastic individual who as a Homecare Nurse, you will deliver gold standard, one to one, holisticcare in the comfort of the their own home. You will be an ambassador for delivering patient focused care, offering quality nursing that is tailored to the patients' needs.

**Duties of the Homecare Nurse**
- You will work as part of a multi-disciplinary team delivering one to one care in line with the patient's needs.
- Maintain professional standards and following the relevant care plan.
- Deliver effective and high quality nursing care to patients in a home environment to facilitate early supported transfer from hospital or to prevent unnecessary admission.
- You will be building lasting relationships with patients, stakeholders and patient support networks to aide and support patients treatment and recovery.
- Accurately document all patient care and clinical observations to provide real time data to assist with long-term goals of the business and the patient.
- As this is a regional role travelling to visit patients in their home setting, it is crucial that you have full UK driving license.

**Knowledge & Experience**
- Registered General Nurse.
- Experience in the care of IV devices and administration of IV therapies.
- Experience of undertaking holistic clinical assessments.
- Experience in complex wound care and wound dressings.
