We are looking to recruit a Hub Co-ordinator to work within the Derbyshire Dales Integrated Community Services team.
The purpose of the Integrated Community Services (ICS) is to provide Nursing and Therapy input to patients in the community in their own homes.
The role of the Hub Co-ordinator is pivotal to the ICS as it is the interface between primary care, secondary care, community care, social care, Therapy, Nursing and GPs in the area enhancing cross service communication and supporting the Joined-up Care Derbyshire movement.
We are looking for an excellent communicator who has the energy, enthusiasm and motivation toimprove people's lives by supporting this integration and service provision.
