An exciting opportunity has become available within the expanding Housing Needs service. We are looking to recruit a hardworking and highly motivated individual to a Complex Needs Case Navigator role on a 12 month fixed term contract.

This new position offers a great opportunity to work within a supportive and high performing team within Adult Services and contribute towards the Government objective of ending rough sleeping.

The Housing Needs service have an established single homeless team that provide outreach support to rough sleepers and the residents of houses of multiple occupation ( HMO's). Blackburn with Darwen has a significant over supply of HMO accommodation which provide housing to individuals with multiple and complex needs, the single homeless team support the most vulnerable residents to help prevent evictions and avoid rough sleeping.

The Complex Needs Case Navigator role offers an opportunity to work as part of the single homeless team and help improve the health and wellbeing of rough sleepers and those at risk of rough sleeping. The role is focused on supporting rough sleepers and residents of HMO’s into drug and alcohol service provision and into structured treatment and harm reduction initiatives. By accessing the support from services individuals will then be better placed to sustain accommodation and avoid rough sleeping in the future.

In this role you will manage a case file of applicants and provide both internal and external data reports which evidence the positive outcomes and lifestyle change that have been achieved.

The successful applicant should have exceptional communication skills and be able to demonstrate the emotional intelligence required to support individuals with complex and challenging needs. Experience of multi-agency and partnership working is also required for this role.
