Our client is looking for the perfect Care Co-Ordinator to join their team who is eager, enthusiastic and who emphasises the importance of accuracy to help a
**TEAM.** Our client have been operating in this location for 5+ years and their success is down to

**CARE CO-ORDINATOR MAIN RESPONSIBILITIES**:

- You will liaise with the team, professionals and clients to optimise the care provided
- You must be capable of building trusting relationships with all contacts
- You will organise rotas, conduct observations and shadow CareGivers when required
- You shall complete care plans/packages
- You will organise service reviews to keep the brand in high regards
- You shall cover calls if carers are unavailable

**REQUIRED SKILLS**:

- Previous experience in Health or Social Care is desirable, but not required
- Extremely organised
- Fantastic communication skills
- Resilient and determined
- Supportive of others
