We have opportunities for highly motivated and innovative band 5 or band 6 midwives to join our highly motivated Tulip continuity team.

The tulip team provides care to pregnancies complicated by diabetes, they work under the direction of the specialist midwife for diabetes. They work flexibly whilst providing a 24/7 cover for intrapartum care.

Each midwife has a caseload of families, providing care across the full spectrum of antenatal, intrapartum and postnatal care in the hospital and in the community. The role offers a varied experience and the chance to work in all areas of the service.

Midwives working in the continuity of carer teams work within small teams to ensure 24/7 birth provision whilst managing their own work. This enables you to meet the needs of your caseload whilst creating a work life balance that best suits your individual lifestyle.

**The post holder will**:

- Provide antenatal (including parent education), intrapartum and postnatal care in all settings based on a team continuity model with a defined caseload.
- Work closely with specialist colleagues and the multidisciplinary teams to deliver choice and personalised care.
- Work autonomously and flexibly managing your own time to meet the needs of your caseload, including undertaking on-calls, while supporting a suitable work-life balance and family care arrangements.
- Contribute to data collection and evaluation relating to the continuity team
- Participate in engagement events both in Trust and the wider community promoting continuity of care

It is paramount that you are a team player and have excellent communication skills, someone who is great at listening to women and their families supporting them to achieve a high quality, positive and safe experience through the development of individualised personalised care and support plans.

You will also work alongside our specialist teams who love helping our team members flourish.

You’ll help support and guide our students and maternity support workers too.

**Duties to include, but not be limited to**:

- Provide professional midwifery care in line with the Nursing and Midwifery Council’s (NMC) Professional Code of Conduct/Rules and Standards
- Develop innovative approaches to care to meet the needs of women and their families.
- Work effectively in all settings providing accessible and flexible services.
- Be accountable and responsible for clinical decision making.
- Ensure appropriate referrals when required
- Provide education to women and families in respect of pregnancy, birth and early parenting skills, including infant feeding practices
- Act as an advocate for women at all times
- Empower women to make informed choices about all aspects of their pregnancy, birth and postnatal care
- Participate in governance and risk management activities including attendance at professional groups and meetings, guideline development, reporting of clinical incidents, research, and clinical audit activities.
