Senior Rehabilitation Assistant

The Intermediate Care beds function as part of the Support and Independence team is to provide a time limited period of assessment, treatment and rehabilitation for adults living within Calderdale or registered with a Calderdale GP. Service users may have had an alteration in their physical or psychological well-being, or a temporary change in their social circumstances which makes it difficult for them to be maintained at home. Alternatively service users may have been discharged from an acute hospital setting and require a period of 24 hour supported living and rehabilitation to enable them to return home.
We are recruiting a full time rehabilitation assistant into our Intermediate Care Beds team to enhance the service currently provided by our registered staff.
We are looking for an individual who is passionate about promoting and facilitating independence in older people with complex needs, to allow them to return to their home environment.
Access to transport is required, and you must be willing to travel across the Calderdale area.
You may be required to work across 7 days, and to support shift patterns between 08:00 and 16.00
You will need access to transport, and you will be required to travel to all areas of Calderdale to allow you to assist registered practitioners with access visits, home visits and supported discharges. You may also be required to transport adaptive equipment.
The post holder will work as part of the wider integrated Health and Social Care Support and Independence Team linking with staff from the acute trust, Primary and Social care.
The post holder will be required to undertake lone working in line with Trust policy.
The post holder will be expected to demonstrate self-directed development and practice.
The post holder will work as part of the multi-disciplinary team comprising Occupational Therapy, Physiotherapy, Assistant Practitioners, Rehabilitation Assistants, Local Authority staff and private provider care staff.
The post holder will undertake skilled support work and may be asked to undertake rehabilitation and support work, including personal care with patients who may have neurological conditions such as stroke, patients who have fallen, respiratory disorders, orthopaedic conditions including post-surgery, amputees, complex multi pathology conditions which may include patients with dementia.
Working to agreed guidelines and protocols and guided by the registered practitioners, the post holder will be key in support of the treatment and care of patients and also their carer/relative where relevant. The post holder will be working under indirect supervision of the registered practitioners in the team. This will involve taking responsibility for an identified and delegated workload following screening and/or assessment by a registered practitioner. The post holder will undertake delegated work following a rehabilitation plan devised by the registered practitioner or assistant practitioner, and will be expected to make minor adjustments to this following agreed protocols and guidelines, liaising with the registered practitioner as necessary.
