As a result of service transformation an exciting opportunity has arisen in Manchester’s School Health service.

We are looking for an enthusiastic and passionate B7 SEND and Transition Project Lead, who is ready to further develop their career by reviewing and expanding our SEND and transition offer for the children and young people of Manchester. The post holder will undertake the lead role for the School Health Service in SEND and transition. The key transition points include from primary school to high school and from key stage to post-16 education. The postholder will be responsible for reviewing and implementing our public health offer and clinical pathways around SEND and transition for children and young people. They will provide advice and support for School Nurses and Health Improvement Practitioner specialists to deliver high quality, evidence-based interventions to children, young people, and their families. They will also oversee the development and delivery of the SEND offer from our Healthy Schools’ Team. This provides health education, resources and training across all schools within Manchester. The SEND and transition post holder will work in partnership to improve the health outcomes of children, young people, and their families.
