This Team Facilitator position is to provide management support to two individuals support services in Edinburgh.

The people being supported require management for their service in a person centred individualised approach which will be met by building a strong relationship with both individuals within their own homes and community.

A key part to this management role is knowing the people being supported well to enable their support team to identify and continually monitor changing health and wellbeing needs, which require to be highlighted and responded to in a timely manner with ongoing support from their manager and health professionals.

Close management and working relationships with the Personal Assistant teams is vital to ensure creative, personalised strategies and outcomes are being facilitated at all times.

This role will require management time spent in both services regularly on a week to week basis to meet the needs of both individuals and their support teams by closely assessing where protocols and strategies need to be reviewed and adapted to implement positive change where required.
