Aftercare Practitioner

As an Aftercare Practitioner with Change Grow Live you’ll work with people who use our services, helping them get to where they want to be.

**Responsibilities**:
The practitioner will be involved with holding cases that are stepped down from structured treatment into recovery support to support with building recovery capital and reduce representations and also to increase group provision and sit alongside the existing Recovery Teams.

The role will work with the BRIC (Building Recovery In Community) lead to establish a programme of events and activities focussing on health and wellbeing, employment and training including life skills such as nutrition and improving access to physical activity and sports.

The practitioner will also have an instrumental role in the development and expansion of a recovery community and peer support network, including in treatment, to sustain long-term recovery, increase the visibility of recovery and support social integration. This could include:

- peer-based recovery support services
- recovery community centres
- recovery support services in educational settings
- facilitating access to mutual aid
- long-term recovery management such as recovery check-ups

**Salary Range (pro rata if part time)**

CGL points 23 to 28 (£26,393.77 - £30,316.74)
