The Social Prescribing Link Worker will work as part of the PCN team to support patients in our local community to take control of their health and wellbeing by
- Promoting prevention & lifestyle changes
- Engaging patients and connecting them with the wide range of groups and services
- Working with the wider health, social care, and voluntary network
- Encouraging a holistic approach for patients with chronic diseases
- Building a relationship in the journey of patients towards mental health services
- Supporting the practices in attaining their referral targets
- Promoting and raising awareness of the social prescribing service within the practices and partner agencies

The SPLW will be part of the PCN Integrated Care Team, led by our Integrated Care Team Lead and supported by our dedicated GP mentor. The multi-disciplinary team currently includes four SPLWs, four Care-Coordinators and three Health & Well-being Coaches, who work collaboratively to deliver a holistic person-centered approach to patients referred to the team by GP practices and partner agencies in the PCN.

The SPLW roles are aligned to particular practices within the PCN to build up established relationships, but also work across the entire PCN to meet the needs of the service. The SPLW team provides a generic service to the PCN patients, but we are also starting to explore targeted patient cohorts for specific focus to address population unmet needs and health inequalities.
