The NWL Virtual Hospital remotely monitors patients from across all NWL acute hospitals in a single team based at St Charles Hospital in Ladbroke Grove. We cover a range of pathways such as COPD, Heart Failure, IBD, maternity & cardiac. We have 19 live wards and are in the process of developing more. Virtual wards are a substitute for in hospital care - this is an opportunity to be part of a move away from traditional models of care,. This is a novel and evolving service so flexibility and a sense of humour are essential. In return, we can offer a flexible and fun working environment, with a flat hierarchy and a commitment to shared decision making.

Current shift patterns are 8am-8pm, 7 days a week. This is an annualised hours post, designed for term time working only. The post holder can work compressed hours (or LTFT), in order to have all school holidays off.NB all staff will be required to work either the Christmas OR New Year bank holiday period. This is a virtual environment, but patient contact remains key. The hub is supported by a network of professional teams, so opportunities will be available to keep up patient facing clinical skills.
- To provide high quality, effective, individualised care to all patients by assessing care needs and developing, implementing and evaluating them without supervision
- To assist the Digital lead Nurse in providing care in a digital remote context and deputise in their absence
- Monitor standards of care and take appropriate action to maintain and improve them when monitoring patients remotely.
- To respond to patients, relatives and carers concerns as they arise and take remedial action as required
- To assist hospital teams in achieving a digital personal health record, helping to ensure the team are able to provide timely review/transfer/discharge of patients.
