We are looking for an enthusiastic team player who is committed to support disabled young people to live their fullest lives. Transition is the process of supporting young people and their families through the teenage years and into adulthood. A good transition should focus on a young person’s strengths and, where possible, work with the young person themselves to identify their goals and aspirations. Transition should also include parents and carers and support them to understand how their rights and responsibilities change as the young person progresses to adulthood.

You will require a thirst to learn about the up-to-date available services in the community such as voluntary agencies and charitable services. Support will be given to expand on your community knowledge; some prior knowledge is desirable.
