We have a fantastic opportunity to come and work for the Major Trauma Clinical Coordinator Team (MTCC). As the Peninsula Trauma Centre, University Hospitals Plymouth has a well-established team of MTCCs.

The MTCC Team are responsible for coordinating the care, rehabilitation, repatriation and transfer in of all major trauma patients while ensuring seamless case management for all Major Trauma Centre (MTC) patients within the Trust. The essential components for this part of the role will include clinical co-ordination and facilitation of safe appropriate care for MTC patients during the acute phase and onwards during the patients stay.

The post holders will need to be enthusiastic, dynamic, excellent communicators with effective influencing and negotiating skills and will be required to work with multi-disciplinary teams across the organisation. Within the MTCC role, a variable shift pattern will be required over a seven day period.

The successful applicant must have recent clinical experience working with Major Trauma patients and good organisational skills. If you are a motivated individual with excellent interpersonal skills looking for a career augmenting opportunity these could be the post for you.

**Preference will be given to internal Trust staff, as well as ‘Priority’ and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.**

The post holder will work in a small group of multi professional Major Trauma Centre Coordinators (MTCC) responsible for case management during the acute phase of care for all patients admitted with major trauma injuries. They will work closely alongside and provide clinical leadership for the data coordinators in the team as well as the governance, training and research and quality improvement leads. The MTC team will be responsible for co-ordinating and overseeing the care for all major trauma patients, including their acute phase of care, through to discharge for rehabilitation or repatriation across the Network and beyond. The MTC Co-ordinators are responsible for the daily running of the major trauma ward round and will liaise with all appropriate/relevant clinical specialties to ensure that all aspects of the patient’s care needs are met whether bedded on the Trauma Admission unit once established, or in other clinical areas. There will be a responsibility to provide support to other clinical staff within the hospital and to provide teaching sessions as needed in relation to the major trauma care pathways incorporating the National Major Trauma Nursing Group (NMTNG) standards as the benchmark for recognised gold standard nursing care in major trauma. There is an expectation that, when available the post holder will attend trauma calls and cover a 7 day rota.

PRIMARY DUTIES & AREAS OF RESPONSIBILITY

1. Assist in the delivery of an advanced level of care for patients attending the Emergency Department (trauma team activations) and patients admitted to the Major Trauma Service to ensure that standards are met, timely decisions are made and that all appropriate specialities are involved in the care of the patient.

2. Navigate the patient through the entire pathway from ED through to discharge from the MTC ensuring compliance with all standards

3. Utilise their in depth knowledge of the care pathway and the complex needs of the trauma patients to provide advanced level clinical coordination of the care pathways from admission to discharge or transfer of care, including liaison with services and to make independent onward referral where required.

4. Support the ongoing assessments of patients including, when required, supporting medical staff in completing secondary and tertiary surveys/assessments and the ordering and follow up investigations for the purpose of diagnosis and treatment planning.

5. Using proactive and skilled approach, undertake specialist and comprehensive assessment of patients’ needs and use advanced clinical reasoning, analytical and investigative skills to interpret clinical and non-clinical information in order to formulate an opinion to inform and influence discharge plans and actively manage the patient pathway (including the completion of patient Trauma Tertiary Survey (TTS)) as outlined in the National Major Trauma measures.

6. Communicate with patients and provide an evidence based rationale for their treatment plans and on-going pathway, supporting them to make informed choices on managing their recovery. Provide up to date information regarding treatment plans and ensure this is reflected in discharge documentation and the rehabilitation prescription.

7. Attend and coordinate the multidisciplinary team (MDT) meetings, Major Trauma ward rounds, MTCC meetings, and any other relevant clinical meetings as appropriate to oversee and promote well organised, multi-disciplinary care for all major trauma patients.
