**30 hours per week, term time only, permanent**

Come and join our fabulous Portage Service in Rotherham Council.

We have an exciting opportunity available; we are looking to appoint a Portage Home Visitor. The post holder will provide a home visiting service to families around Rotherham with children (0-3 years) with special educational needs and/or disabilities. To carryout weekly home visits where the Portage Home Visitor provides the Service and to maintain records of the child’s skills using the Portage checklist or other appropriate tool.

The post holder will support and encourage early years services to work together so that young children with additional needs make good progress, ensuring families access relevant services and enabling a smooth transition into formal education.

They will contribute to holistic assessments including consideration of how early childhood development, life experiences and learning style is affecting the child’s progress.

The role will need to establish effective links with settings and schools to ensure a joined up approach to assessment and provision in which liaison between home and setting is key.

They will deliver training to families and practitioners.

They will need to have extensive knowledge and understanding on all aspects of early years and will need to maintain this throughout the role.

The post involves outreach support therefore an enhanced DBS will be required. Some occasional and evening work will be required.
