Responsibilities:

- Provide direct care and support to individuals in a community setting
- Assist with daily living activities, including personal hygiene, meal preparation, and medication management
- Develop and implement individualized care plans based on client needs and goals
- Monitor and report changes in client's physical and emotional well-being
- Collaborate with a multidisciplinary team to ensure coordinated care
- Advocate for clients' rights and promote their independence and inclusion in the community
- Maintain accurate and up-to-date documentation of client interactions and progress

- Strong organizational skills and attention to detail
- Ability to work independently as well as part of a team

**Job Types**: Full-time, Part-time, Permanent

**Salary**: £10.75-£11.00 per hour

Expected hours: No less than 16 per week
