An opportunity has arisen for an enthusiastic and motivated individual to join our Telford Locality team.

The service consists of multi-disciplinary team including Physiotherapists, Occupational Therapists and Rehabilitation Assistants, also working closely with our nursing colleagues and social care practitioners to provide a patient centred approach. The aim of the team is to promote independence through rehabilitation and crisis management with patients in their own home or within a short term care setting to prevent unnecessary hospital admissions and support timely hospital discharges. We can then provide on-going rehabilitation within the patient place of residence to improve, maintain or help to manage underlying conditions. Our aim is to optimise everyones potential to gain greater independence to add quality to their life.

The role includes delivery of efficient and effective services in line with local, national and professional agendas.
