Are you passionate about Facilitating and Supporting patients on discharge from hospital ?

Integrated Supported Discharge Team ( ISHDT) are looking for the right person to join our highly valued team in providing therapy assessments and interventions for patients in their own homes after being discharged from acute hospitals.

We are looking for an enthusiastic Senior Rehabilitation Practitioner who can:

- Provide a high standard therapy service to patients upon discharge
- Support the multi professional team in coordinating referrals
- Work effectively in a team environment
- Be able to travel efficiently across the borough and all sites within NELFT

The post holder must have previous experience in providing rehabilitation and be able to work flexibly and effectively as part of the multidisciplinary team, covering a 7 day rota.

You will be working as part of a multidisciplinary team consisting of Physiotherapists, Occupational Therapists, Social workers and Social care assistants. The team is an integrated team between health and social care, which will enable you to gain a wide range of skills in rehabilitation, coordinating discharges and advising on level of support/care needs of service users.

Probationary Period

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,108 and a maximum payment of £5,177 per annum pro rata).
