Job Description: Support Living Worker

Location: East Midlands

**Salary**: £27,000 per annum

Number of Positions: 20

Additional Requirements: Ability to work shifts, including weekends, some nights, and Bank Holidays, due to 24/7 support requirements.

Key Responsibilities:

- Person-Centered Support: Provide exceptional support to individuals with learning disabilities, aligning with their unique support plans, cultural preferences, and wishes.
- Home Management: Assist individuals in managing their homes, including supporting them with tenancy maintenance and guiding them during home transitions.
- Personal Care: Offer sensitive and respectful assistance with personal care tasks such as toileting and bathing, adhering to individual support plans.
- Empowerment and Choice: Encourage and facilitate individuals to make informed choices and life decisions in compliance with the Mental Capacity Act.
- Engagement and Participation: Foster engagement in daily home, social, leisure, and work activities based on the individuals' preferences and choices.
- Health and Well-being: Provide necessary support for health-related needs, such as medication management and attending medical appointments.
- Accurate Documentation: Maintain accurate and up-to-date records to ensure comprehensive and effective support.
- Financial Guidance: Help with managing finances in accordance with Hft guidelines, promoting financial well-being.
- Safety and Well-being: Prioritize the health, safety, and overall well-being of the individuals under your care.
- Collaborative Approach: Foster positive relationships and collaboration with colleagues, families, external organizations, and professionals.

Qualifications and Requirements:

- Experience in providing support to individuals with learning disabilities, preferably in a residential or community setting.
- Compassionate approach and commitment to delivering high-quality care.
- Willingness and flexibility to work shifts, including weekends, some nights, and Bank Holidays, due to the 24/7 support nature.
- Strong communication skills to interact positively with individuals, colleagues, families, and external stakeholders.
- Familiarity with the Mental Capacity Act and its principles.
- Empathy, patience, and a positive attitude toward promoting independence and choice.
