**Job overview**:
The Integrated Support Worker within Integrated Neighbourhood Team will provide care and support for patients across the community setting of Heywood, Middleton and Rochdale. The post holder will participate in the delivery of care and therapy interventions and delegated tasks without direct supervision, reporting to a Registered Practitioner.

The post holder will be a member of the Integrated Neighbourhood Team, with clinical skills and competencies achieved via a national Care Certificate or NVQ level 3 in Direct Care (or be willing to work towards) and further identified training. They will assist in the delivery of high standards of health care supporting patients within their community setting by following prescribed packages of care.

**Achievement of national Care Certificate / L2 Health related vocational qualification, or willingness to work towards is required for this role.**

**Main duties of the job**:
The individual in this role will actively participate in providing care, therapy interventions, and assigned tasks without direct supervision. They will report to a Registered Practitioner.

Their primary responsibility will be to support patients within the community setting by delivering healthcare services that adhere to established care plans. This includes assisting in maintaining high standards of healthcare.

The post holder will carry out delegated duties under the supervision of registered professionals. They will also recognize the need for referrals and collaborate with other healthcare, therapy, and multi-agency workers as required. Effective communication with all disciplines within the Trust and external entities is essential.

Furthermore, the individual will provide support to staff in managing and executing clinical practices. They will maintain accurate and legible documentation, following prescribed standards and protocols.

The post holder will manage their own caseload of patients with ongoing management and complex needs. They will offer enabling support and care to patients experiencing changes in their health or function, aiming to prevent unnecessary hospital admissions and ensuring smooth transitions during discharge.

- Ability to demonstrate an understanding of the rehabilitation process
- Able to organise own workload and meet deadlines
