An exciting opportunity has arisen at Whitchurch Community Hospital, for an Advanced Nurse/Clinical Practitioner as we develop and increase our Medical Team.

We are looking for an experienced, dynamic person to join our well established multidisciplinary team, working in a rural setting where the hospital is at the heart of the local community.

The post holder will be an autonomous practitioner with an appetite for providing consistent senior clinical leadership and medical cover for older people with chronic conditions, frailty and end of life care.
- 37.5 hours per week

The ward environment is busy and demanding, you will be expected to display a high degree of professional autonomy and be the sole decision maker for inpatients on the ward.

The ACP will assess and treat from a range of previously undiagnosed differential diagnoses, formulating treatment plans to manage patients in the community setting.

The post holder will be responsible for comprehensive and advanced physical assessments of patients and for requesting and interpreting diagnostics in order to deliver expert clinical reasoning and clinical decision making.

The ACP will be responsible for delivering advanced interventions to patients including cannulation, line insertion and management and the administration of sub cutaneous and intravenous therapies.
