**About The Role**

You will be providing support to customers across Conwy in a variety of locations, such as private lets, own homes and temporary accommodation, with the aim of supporting them into longer term permanent accommodation to meet their needs.

You will:

- Provide an effective tenancy support service across a range of tenures to an agreed caseload of service users. This includes clients with complex needs to enable tenancy sustainment or to assist service users to access appropriate new accommodation where they are living in unsuitable temporary accommodation.
- Receive referrals from Local Authority and conduct a detailed assessment of need and risk to develop an outcome focussed and goal orientated support and risk management plan, as specified in the contract. This will support the Senior Project Officer to meet contractual requirements and reporting obligations.
- Deliver housing related support and conform to Supporting People Guidance, standards and eligibility criteria.
- Escalate concerns related to service users to the attention of the Senior Project Officer for guidance and support.

**This role requires an Enhanced Disclosure & Barring Service (DBS) check which we pay for.**
