An exciting opportunity has arisen to join Blaenau Gwent Community Resource Team in Aneurin Bevan University Health Board as a Consultant Geriatrician with an interest in Intermediate Care. This is a permanent position and will be community based.
As a key member of the Community Resource Team, the Consultant will be responsible for leading and developing our Rapid Response service and for ensuring that the highest standards of care are delivered to patients in their own homes.
The team has a positive reputation for providing responsive, high quality care and comprehensive geriatric assessments for older people, which are consultant led. We have outstanding services led by our Doctor, Nurses, Occupational Therapists, Physiotherapists, Assistant Practitioners and Support Workers who constitute our rapid response, discharge to assess, falls and reablement teams.
This is an exciting post designed to improve patient experience through providing care locally as part of our Clinical Futures and Care Closer to Home Strategies. The post holder will be joining a passionate and dedicated team with a strong ethos for delivering community based care to frail elderly patients in partnership with Blaenau Gwent Local Authority Social Services, Primary Care Neighbourhood Care Networks, Community Hospital and other local organisations.
