IntraHealth is one of the UK’s leading provider of NHS Primary and Community Care services. We serve a range of patients across our three divisions of Primary Care (general practice), Pharmacy and Clinical Services which includes anticoagulation monitoring, patient medication reviews and childhood immunisation programmes.

We also provide management and clinical support to other GP practices and NHS bodies. We are a well-established organisation having provided NHS services since the company was founded in 1999. Our team is made up of GPs, Advanced Nurse Practitioners (ANPs), Nurses, Pharmacists, Pharmacy Technicians, Health Care Assistants (HCAs) and local administration teams supported by a centralised back office function; providing finance, HR, administration and data analysis support.

We operate NHS services across the North East, North West and Yorkshire.

IntraHealth are looking to recruit to the post of Care Coordinator, to work at our West Cornforth Medical Practice.

They will work closely with GPs and practice teams, making sure that appropriate clinical support is made available to people.

The Care Coordinator would also signpost patients to the most suitable external organisations who could support with issues that were less directly health
- related, such as citizens advice, wellbeing for life, exercise referral, community groups etc.

They will work alongside social prescribing link workers and health and Wellbeing for Life to provide an all-encompassing approach to personalised care and enable people navigate through the health and care system.

The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a varied and sometimes challenging environment is essential.

Care coordinators play an important role within a practice to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

Care coordinators help patients negotiate their way through the NHS and so can help them organise secondary care follow up or appointments within the wider NHS. They will also coordinate community care of complex patients after their admission to hospital. Their aim is to help people access the healthcare they need across all settings.

Care coordinators play a pivotal role within practices to coordinate the call and recall system of people with long-term conditions to ensure people attend vital follow up within the practice.

Care coordinators will attend multidisciplinary team meetings within the practice to ensure appropriate follow up for patients occur.

They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families, and carers with high quality support.

Please note that the role of a care coordinator is not a clinical role.
