**Job summary**
The practice is housed in purpose-built premises, alongside the local community hospital. Multi-Disciplinary working is at the heart of what we do, with both district nurse and ambulance teams on site here with the practice. The role offers the opportunity to work remotely providing primary care, as well as be involved in working closely with the local community hospital, which offers an interesting portfolio aspect to your future role. We are a well-established practice that cares for a population of 8,500 patients and we have been rated as overall "Outstanding" by the CQC.
**Main duties of the job**
The post holder will act as an autonomous practitioner working independently within their professional boundaries to assess, diagnose and treat the conditions of patients You will provide expert advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
The post holder will be responsible for promoting and facilitating a healthy lifestyle and provide preventative health care and health education advice. The role will primarily consist of telephone and face to face appointment providing advice and clinical assessment when required. You will also be required to action and request tests such as bloods, x rays, letters and ultrasound scans as able.
The post holder will take a lead role for diabetic patients within the practice initiating and monitoring their dietary and medication needs. The post holder would alter medication doses when required in keeping with any blood test results.
The post holder will provide leadership the clinical team on a day-to-day basis, and to assist with the transition/change management required. Provide support and assurance to the clinical team members.
**Duties and Responsibilities**
Support the Management team to ensure all QUOF Targets are met
Facilitating and undertaking Clinical Audits, cascading lessons learnt and ensuring clinical practice is improved
Practice autonomously, as an independent practitioner/prescriber, participating in the care of and undertaking assessment, diagnosis, treatment / referral, and discharge of patients (Adults and Children) presenting with minor illness or injury within agreed parameters
Provide advanced clinical skills and advice demonstrating a sound understanding of the issues related to identification, assessment, diagnosis, treatment, and management of a client group whilst working autonomously
Identify the need for and initiate and interpret relevant diagnostic investigations and results
To safely and competently undertake invasive and non-invasive procedures to establish a diagnosis
To work within the parameters of current nurse prescribing legislation
Monitor and lead improvements to standards of care through supervision of practice, clinical audit, and evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership
Act in accordance with the NMC Code of Professional Conduct
Provide professional and clinical advice within the multi-disciplinary team
To act as a mentor to allocated clinical staff
To perform IPRs with allocated clinical staff
Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
Prioritise, organise, and manage own workload in a manner that maintains and promotes quality
Deliver care according to local and national guidelines
Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making
Work within all relevant policies and procedural guidelines e.g., infection control, chaperoning and risk management
Safeguarding responsibilities for adults and children and vulnerable groups, reporting concerns appropriately at the correct time and engaging with services as needed. The post holder will act as the deputy safeguarding lead for Waterloo House.
To answer and fully investigate clinical complaints with support from the Clinical Manager
The Head of nursing may also ask for other duties to be carried out to the benefit of the organisation.
**Clinical practice**
Autonomously assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and those with long term conditions in line with national and local policies and practice/ network guidelines
Diagnose and manage both acute and chronic conditions, integrating both drug
- and non-drug-based treatment methods into a management plan
Prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
Work with patients to support concordance with and adherence to prescribed treatments, provide information and advice on prescribed or over-the-counter medication on medication
