To work closely with the Wellbeing and Care Teams in identifying areas of need and support for patients, their carers and wider family members
- To liaise with other organisations in the provision of Wellbeing activities and support for families
- To coordinate family support based on a holistic needs assessment
- Deliver an empowering, flexible, and integrative style of person-centered care, to adults, families, children, and young people who have faced illness and bereavement
- To provide support throughout the transition pathway for young people transitioning to adult services as required
- To work with families, including in their own homes, groups and on a one-to-one basis to increase the skills, confidence, and abilities of all family members to live their daily lives in a positive and safe way
- To build up knowledge and understanding of local resources and community and statutory services, including Mental Health Teams, Social Services, Health, EWMHS, Schools and voluntary services, and communicate effectively with them in the best interests of the patient, carers and family members
- To work as a key member of the Wellbeing Team to ensure high quality service delivery to all patients, carers, and wider family members ethically and effectively in line with organisational and professional standards
- To be responsible for a family support caseload as defined in conjunction with, and approved by, the Hospice Social Workers and/or Wellbeing Manager / Head of Wellbeing
- To carry out home visits and complete holistic needs assessments
- To plan and implement appropriate sessions for identified areas of support, using professional judgement and family engagement to determine the best support plan to meet the need
- Use of professional judgement to analyse complex information and make decisions in the best interest of the patient, carers, and family members
- To participate, where appropriate and possible, in helping to organise any further support or social groups, where the need is identified within the Wellbeing Team

Our specialist Care Teams can support them and their family living throughout illness, death and bereavement in the comfort of their own home and through our hospice services, Fair Havens and Little Havens.

- Be flexible and creative in the support of the patient, carers, and family members
- Maintain accurate records of patient care using SystmOne following Havens Hospices policy ensuring that personal caseloads are maintained in line with the record-keeping policy
- Review quality of interventions and ensure patient/family satisfaction, following up on comments and concerns as applicable
- Be flexible in working patterns due to the needs of patients and families
- Use of professional judgement to analyse complex information and make decisions in the best interest of the patient, carers, and family members
- Observe personal duty of care concerning equipment and resources used in course of work
- To manage own wellbeing with the support provided due to frequent exposure to patients and families in distress
