**Social Prescriber Link Worker (On Duty Triage)**
**EMPLOYED BY**:High Peak CVS
**LOCATION**:Combined Home, Office and Remote working:Office Location: Whaley Bridge, High Peak, SK23 7HX and to work flexibly across eight GP Surgeries.
**WORKING HOURS**: 28 hours per week
**RESPONSIBLE FOR**: To deliver a Social Prescribing service across the High Peak
**REPORTING TO**: Social Prescribing Manager
**SUMMARY OF POST**:
Social Prescribing aims to increase people’s active involvement in their local communities. It can strengthen resilience at both a personal and community level, and it reduces health and wellbeing inequalities by supporting people to address the wider determinants of health such as; debt, poor housing and physical inactivity, including vulnerable adults. Particularly for High Peak, it works for people with long term conditions (including support for low level mental health) and those people who are lonely or isolated.
As part of this initiative our Social Prescribing service have run a pilot to triage appropriate referrals and created an On Duty role to manage this.
This role will co-ordinate, through a Triage system, the Social Prescribing service referral process, across the High Peak by monitoring the appropriateness of the patient referrals from GP surgeries and beyond as we open up to outside agencies and self referral, in the High Peak Primary Care Network.
This role will work closely with the Social Prescribing Manager to discuss complex cases and develop a stepped approach with other statutory service providers.
This role will be responsible for developing good collaboration with each GP practice to monitor and improve the appropriateness of the referrals.
This role will also be responsible allocation and case load monitoring for the SP team. with appropriate services and support from statutory, local community and voluntary non-profit sector, service providers.
The role will be responsible for gathering feedback from clients by phone, encouraging clients to be honest about their experience with the Social Prescribing service.
This role will play a key part within the SPLW team lone working process during home visits.
**MAIN DUTIES AND RESPONSIBILITIES**
- Monitor referrals and assess appropriateness and accuracy of the referral form
- Manage obtaining further information from referrer's or other AHP team's as appropriate.
- Refer back with recommendation (RBR) if the referral is inappropriate or best placed in another team/service.
- Recognise when to seek support form Social Prescribing Manager to manage complexities and make decisions re; RBR.
- Support the Social Prescribing Manager to allocate cases, communicating with the wider SPLW team information they need to know about incoming referrals.
- Attend regular meetings to answer questions and provide guidance regarding complexities to other SPLW's in the team.
- Develop partnership working with GP's and practice staff for each practice.
- Make contact with new referrals, ensuring they have been provided information about the Social Prescribing service in a timely manner.
- Provide low level active signposting, where appropriate, to clients when assessing and contacting incoming referrals.
- Develop trusting relationships and taking a holistic approach, based on the person’s priorities and the wider determinants of health.
- Be creative in finding solutions that harness assets and resources which are already available and are low cost/non funding-dependent.
- Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to the referrer with recommendation or to statutory organisations.
- Work with the group development team at HPCVS to draw on and increase the strengths and capacities of local communities, enabling local voluntary organisations and community groups to receive social prescribing referrals.
- Support the local mapping of community based services, assets and services in support of personalised care available, to facilitate independence and build on individual capacity and resilience.
**REFERRALS**
- Assess all incoming referrals from partners, external agencies and individuals, ensuring the Social Prescribing service is the most appropriate form of support for the client.
- Provide active signposting to clients who may only need information without the additional support providing the wider SPLW team.
- Promoting social prescribing, its role in self-management, and the wider determinants of health.
- Build professional relationships with key staff in GP practices within the local Primary Care Network (PCN).
- Attend relevant meetings such as; CMHT MDT's to become part of the wider network team, giving information and feedback on social prescribing.
