This a pivotal role in anticipatory care and the administrative post holder will have a key role to fulfil within the Frailty and Anticipatory Team. The Integrated Network Coordination (INC) Administrator will provide a flexible and responsive service to the Integrated Networks. The INC service is part of Frailty and Anticipatory Care Team in Islington and implement the anticipatory care approach in reducing hospital admissions. The post holder will provide high quality service to patients by supporting the Integrated Networks. The post holder will also deliver efficient, effective, quality and patient focused service across Islington.

He/she will provide related general administrative support to the INC service. He/she will support the INC Team Lead and Service Manager in monitoring and tracking performance against relevant key performance targets within the new services providing a new way of working. The post holder will liaise with all staff to assist the patient’s journey.

Manage and support the team in identifying appropriate patients for MDT intervention as well as supplying lists of other patients identified for other levels of care, as required.

Where patients are identified for MDT intervention, the INC Administrator may be involved in obtaining informed consent and assisting service users to identify their health and social care goals, prior to discussion at the MDT teleconference or Network meeting.

Coordinate and manage the administrative functions for Integrated Network MDT meetings. This will include organising and adjusting meetings and tasks on behalf of a number of stakeholders, coordinating “face to face” or “virtual” meetings and recording minutes and actions from the meetings and patient discussions.

Be the point of liaison for service users and carers receiving care coordination and interface with all professionals involved in service users’ care, including primary, secondary, London Ambulance Service, community services, voluntary services, carers and other relevant groups e.g. Telehealth

Medical services have been delivered on the Whittington Hospital site for over 500 years. On 1st April 2011, Whittington Health was launched as a new Integrated Care Organisation (ICO) comprising The Whittington Hospital NHS Trust, NHS Islington and NHS Haringey. The Whittington and the community health services in Islington and Haringey are award winning organisations, delivering acute and community based health services to a population of 443,000 people.

The organisation is the biggest employer in the area, with over 4,500 staff; delivering healthcare not only on the acute hospital site which has 420 beds, but also from a variety of other community based health centres and through visits to patients homes across the boroughs to address the health needs of the local population. Whittington Health also works with other health, social care and voluntary sector partners to support patients and service users from their initial appointment whether it be with a community health team or at the hospital, all the way through to treatment and tailored after care.

Deal in a professional, helpful and sensitive manner with service users, staff and other agencies by telephone or face to face, taking messages, advising service users about home visits, referring other issues as appropriate and answering routine enquiries.

To be proficient in using Microsoft office packages and other identified IT systems and support other team members in their use.

To extract caseload information from the service user database as required, and undertake analysis of caseload information for audit and performance management purposes, including running reports and converting data into charts and tables for sharing with the team.
