**What you will be responsible for**
- Working within a locality service and through participation in the local referral and care pathway, provide clinical assessment and appropriate triage with a view to risk assessment, risk management, safeguarding, sign posting, treatment or onward referral in response to identified patient need.
- Provide front line assessment to clarify the services the patients would most benefit from. This assessment may be undertaken jointly with other providers where it would be of benefit. This may be with the GP or with other statutory or non-statutory member of staff.
- Where referrals indicate, work jointly with the local mental health services, specialist mental health teams and non-statutory teams to support prompt assessment and, where clinically indicated, appropriate treatment plans.
- Design, develop and provide service information about the options available to patients.

The role will involve, but not limited to you performing the above.

**You will have the following qualification/experience**:

- A minimum of 6 months paid experience as a Community Mental Health Practitioner for a UK hospital environment within the last 3 years
- Comply with NMC Professional Registration and keep up to date with current nursing practices
- Ensure accurate records are kept in accordance with CHS policy and NMC guidelines
- Undertake regular mandatory training*
- Full UK driving licence and own vehicle.

**Depending on the Trust, you may require variable additional certification and skills.**
