We are pleased to be seeking 1 Full-time (37.5 hours) position in Huntingdon.

We are looking for dynamic RNs or Paramedics with amazing interpersonal skills to join our 2 - 4 hour admission avoidance service.

JETs aim is to prevent unnecessary hospital admission by providing high quality holistic assessments and diagnosis managing patients with sub-acute conditions to aid their recovery within their own home.

We are looking for experienced clinicians with a passion for a high quality care and experience in either acute medicine emergency medicine or community.

You will work alongside colleagues with real expertise in this area of the community care. The team has a passion for improvement innovation and delivery of a high quality of care to patients in their own home and working within the wider multi-disciplinary team within the local community.

Working across the county visiting patients within their usual place of residence you will be a valued member of the service.

**The purpose of the role**:

- To provide a high quality standard of care to patients who have a wide range of physical needs affecting their ability to maintain a level of independence.
- To deliver patient centered care that empowers individuals to achieve their optimum physical, psychological, social and emotional wellbeing.
- To provide a holistic and clinical assessment, investigate, diagnose and implement patient centered care working in partnership with the MDT.
- To provide advice and support to patients, families, carers and voluntary agencies regarding the patient’s medical condition ensuring that an effective communication pathway exists.
- To provide a holistic person centered approach to care that meets the specific needs of individuals by enabling an increase in their independence, improving health outcomes and reducing hospital admissions or long term care

1. Ensure there are robust and strong lines of communication with GP, MDT, Ambulance Service, Neighbourhood teams, as well as the wider team.
2. Ability to communicate complicated and sensitive information in an appropriate manner to patients, families and carers promoting understanding whist recognising barriers to communication that may exist (e.g. hearing impairment, literacy)
3. Be aware of physical barriers to patient and carers understanding, due to intellectual, cultural or physical limitations and work to overcome these.
4. To negotiate with carers/clients and others around individual case management, particularly in relation to complex cases.
5. To recognise potential breakdown and conflict when it occurs, seek advice and support and demonstrate established negotiation skills in the management of conflict.
6. To establish professional relationships and communicate complex information to other health, education and social care professionals involved with the patient
7. To demonstrate the ability to form productive relationships with others who may be under stress/and or having challenging communication difficulties.
8. To ensure competency in medicine management with skills kept up to date and carried out in compliance with policy and procedures
9. To demonstrate care and compassion in your work
