**Directorate: Adult Care, Housing and Public Health**

**Temporary for 12 months, full-time, 37 hours per week.**

Base: Flexible working from Riverside House

We have an exciting opportunity to make a difference with people with a dual diagnosis and be at the forefront of service change and delivery. The postholder will work within the Complex Lives Team who provide a holistic service offer that is person centred and asset based. Focused on recovery, resettlement, empowerment, and inclusion, which emphasises prevention and early intervention, managing demand through proactively engaging, supporting people living complex lives.

These are people with a combination of mutually reinforcing challenges including homelessness, drug, and alcohol misuse, offending behaviour, mental ill health, poor physical health. People in these situations have often experienced childhood trauma, family breakdown, domestic abuse, and other major life changing events.

The Postholder will:
Possess sound assessment and care management skills and the ability to communicate effectively and use your own initiative where appropriate.

Work in partnership with other professionals to deliver a high standard of risk management and social work practice

Possess demonstrable experience of working with individuals, family, and carers to identify people’s strengths, networks, and community assets.

Work collaboratively with staff across the structure to identify existing gaps and opportunities related to social work professional practice

Ensure and contribute to the development of excellent health/social work practice.

Hold a portfolio of priorities linked to the dual diagnosis pathway

Develop best practice and lessons learnt and proactively providing educational and practice development activities to staff.
