**Job summary**
The PCN Pharmacy Care Co-ordinator may be required to deal with patients and, if appropriate, their carer, before or after the patient’s consultation with a clinician or other healthcare professional.
Working closely with the patient, their clinician and Pharmacy Team, the Care Co-ordinator helps to co-ordinate patients’ health and social care, and directs them to the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.
The Pharmacy Care Co-ordinator’s role requires them to be able to work with, and understand the roles of, a variety of different people working across Farnham PCN including GPs, Practice & Community Nurses, Advanced Nurse Practitioners, Healthcare Assistants, Social Prescribing Link Workers, Physiotherapists, Physician Associates, Paramedics, Health and Wellbeing Coaches, Occupational Therapists, Clinical Pharmacists and Pharmacy Technicians.
They may be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification.
They will need to support and promote health, equality and frailty (elderly care administrative support)
**Primary responsibilities**
The Care Co-ordinator will work as part of the Personalisation & Anticipatory Care Team (PACT), consisting of Social Prescribing Link Worker, Health & Wellbeing Coach, and Dietician. Clinical leadership & support will be provided by the PCN Clinical Director (or their nominated lead(s) for proactive/integrated care, population health management, and health inequalities). The role has the following aims:
- To identify and work with cohorts of people to support their personalised care requirements, using any available population health management and decision support tools, templates and software
- To collate patients’ identified care & support needs, review the options to meet these needs, and bring them into a single personalised care and support plan (PCSP) in line with best practice
- To help people to manage their needs by answering their queries and supporting them in making appointments, accessing appropriate benefits, taking up employment & training, and, where appropriate, to assist patients to access personal health budgets
- Liaise with community and hospital pharmacy colleagues to help resolve medication related issues.
- Develop and maintain medicines related communication to all Practice staff
- Assist with the development and review of medicine audits
- Support with project work/prescribing initiatives/clinical audits under the support of the Pharmacist
- Invite patients to attend required blood tests
- Attend local meetings seeking to improve access to medicines or repeat prescribing processes.
- Support the PCN multi-disciplinary team to ensure efficient medicines optimization processes are being followed.
- Maximise safe, cost-effective, prescribing to improve the quality of patient care.
- Support Quality and Outcome Frameworks and other DES/LES specifications
- To assist patients to be better prepared to have conversations on shared decision making and to improve awareness of shared decision making and related support tools
- To provide patients with high quality, easy to understand information to assist them in making choices about their care
- To support patients in understanding their level of knowledge, skills and confidence (known as “activation” level) when participating in their health and well-being using, where appropriate, the Patient Activation Measure
- To assist patients to access self-management education courses, peer support or interventions that support them in their health and well-being
- To provide co-ordination and navigation of patients, and where appropriate their carers, across health and social care services
- To support in the delivery of enhanced services and other service requirements on behalf of the PCN. Undertake work in line with PCN directed priorities.
- Work with our clinical pharmacist to ensure patients are managing their medication safely and support the safe, accurate and timely prescribing of medication
- Undertake medicines reconciliation in accordance with practice protocols and to update patients’ medical records, including addition/removal of medications.
