Key Responsibilities:

- Coordinating and scheduling care services for clients in their own homes
- Supervising a team of care assistants and ensuring that they are providing the highest quality of care to clients
- Communicating with clients' families and healthcare professionals to ensure continuity of care
- Assessing clients' needs and developing care plans to meet those needs
- Monitoring clients' progress and making adjustments to care plans as necessary
- Ensuring that care assistants are following care plans and reporting any changes in clients' conditions
- Managing client referrals and admissions to the service
- Ensuring that all documentation and paperwork is completed accurately and in a timely manner
- Providing Care in the community when required

Qualifications:

- Must have excellent communication and interpersonal skills
- Must be highly organized and able to multitask
- Must be able to work independently and as part of a team
- Must have a valid driver's license and access to a vehicle

Previous care experience is preferred.

**Job Types**: Full-time, Permanent

**Salary**: £27,000.00 per year
