**JOB TITLE**: Health Care Assistant (HCA) REPORTS TO: Operational Manager HOURS: 15 - 25 hours per week, with opportunity to work in extended hours hubs on Saturday. There could also be opportunity for full-time hours. SALARY: £11.95 per hour CONTRACT TYPE: Permanent LOCATION: Northern Medical Centre, Barnsbury Medical Practice Extended access hubs across Islington, as appropriate VACANCIES: x 2 The Role: Barnsbury Medical Practice (BMP) and Northern Medical Centre (NMC) are managed by Islington GP Federation (IGPF). Islington GP Federation (IGPF) is looking for 2 Health Care Assistants for an innovative position working across these practices.

Our current range of services include the Extended Access Service, I: HUB, Community Ear, Nose and Throat (ENT), Integrated Community Gynaecology, practice-based pharmacists and a range of practice support mechanisms. IGPF works very closely with a range of partners including the regional commissioning group, NHS England, Healthy London Partnership, Public Health, local hospitals such as UCLH, the Whittington Health and the London Borough of Islington. IGPF is the host organisation for the Primary Care Network (PCN) workforce and the Islington Training Hub and has been working for over two years to create training and development programmes that meet the needs of staff working in primary and community care settings. IGPF runs two Islington GP practices on caretaking contracts; a third practice will be taken on within this financial quarter.

Job Responsibilities Request and follow up blood tests Perform wound care and venepuncture Record and monitor vital signs, recognise abnormalities, and escalating to a senior when needed. Perform ECG recording. Maintain accurate records of patients conditions, care received and given Handle enquiries, concerns and complaints from patients and visitors in a professional manner Conduct physical health checks; Phlebotomy, peak flow readings, blood pressure, and weight & lifestyle Conduct checks for metabolic diseases; diabetes, chronic kidney disease, and hypertension Conduct respiratory conditions checks; asthma and COPD Contribute to discharge process Assist with preparation of clinical procedures and clearing environment afterwards as appropriate. Personal/Professional Development: The post-holder will participate in any training programme implemented by the Federation as part of this employment, such training to include: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
