Therapy Assistant Nutrition and Dietetics

**Working Part Time**: 30 hours per week
We are looking for an enthusiastic individual to join our nutrition and dietetic team. This person should be friendly, motivated and enthusiastic, with a positive can do attitude who can learn and adapt quickly.
This person will work predominantly within the Diabetes Essentials team but also provide support to the PCN Dietitians.
Main duties include the delivery of Diabetes Essentials - in groups and 1:1 setting, support referrals to other agencies and research into barriers of structured education via patient contact and EMIS data.
- You will work across hospital, GP practices and various community settings.
- Applicants should have a professional and pleasant manner at all times.
- The post holder will require access to a car and a clean valid UK driver’s license as they will need to carry out community visits.
- Previous experience of EMIS and Diabetes would be advantageous.
- There are excellent IT facilities in all offices, including internet access and we are committed to training and professional development
- Run EMIS identify patients that have not otherwise been offered diabetes education.
- Improve relationships with individual practices and support their referral pathways to diabetes education.
- Identify patients with both diabetes and other risk factors that could result in health inequalities, ensure they are directed to appropriate tailored support
- Direct contact with the patients to discuss the benefits of attendance and scheduling of 1:1 appointments.
- Book patients to both structured education and signpost to additional health improvement services.
- Supporting the input of SNOMED CT structured education codes to improve the accuracy of the National Diabetes Audit data.
The Countess of Chester Hospital NHS Foundation Trust consists of a 600 bedded large District General Hospital, which provides its services on the Countess of Chester Health Park, and a 64 bedded Intermediate Care Service at Ellesmere Port Hospital.
