An exciting opportunity has become available to join our Digital Health Virtual Wards Team as a Health Care Assistant.

Virtual Wards offer enhanced healthcare at home to enable people to receive care at home as an alternative to hospital admission. We use a technology to be able to remotely monitor people as well as complete face to face visits and wrapping existing community services around individuals

Working as a member of the Virtual Ward team you will review patients virtual observations and set clinical questions using a remote monitoring platform. You will be required to complete home visits to patient on the virtual ward. You will be responsible to escalate any concerns to senior clinicians.

A good understanding and ability to use IT is essential for this role.

Patients monitored on the ward will be either Respiratory or those living with Frailty.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
