**About The Role**:

**Contract**:Fixed Term Contract until 31st December 2024.

**Location**:Community based in West Wales - Carmarthenshire with some homeworking elements.

**About the role**

This Dementia Adviser role will be part of an exciting new early intervention service in Carmarthenshire. The Early Intervention service will provide time-limited support, information and guidance to people affected by dementia (including family and carers) pre
- and post-diagnosis.

The Dementia Advisers in this service will role manage volunteers to promote dementia awareness, and the importance of getting a diagnosis across the Carmarthenshire area.

The post-holder must live within (or near) the Carmarthenshire area in West Wales. There is an expectation to travel for home-visits, and occasionally to community events to promote the service and engage with local partners.

**About you**

You will need to be determined to make a difference to the lives of people affected by dementia across Carmarthenshire.
- We are looking for someone with a person-centred approach to support, guidance, and information provision to empower people affected by dementia within the early stages after diagnosis.
- This can be an emotionally challenging time for individuals and families, so you will be compassionate, understanding, and professional.
- You will work well providing time-limited support to people, empowering them to make decisions and plan for the future, while living well with dementia in the present and being engaged in their communities.
- You will need to confident to network and collaborate with internal and external colleagues to promote the service and create links with services that support people affected by dementia in the community.
- We are also looking for someone able and willing to role manage volunteers, who will raise awareness of the importance of getting a diagnosis across Carmarthenshire.

*Ability to speak Welsh is desirable for this role*
