**Sat/Night £28.02 per hour**

**Sun/Bank Hols £28.02 per hour**

Working hours: 09:00-17:00 Monday
- Friday

CSH is one of the 11 health and social care providers who are known as the Surrey Heartlands Health and Care Partnership. We work together to improve care across the Surrey Downs, North West Surrey and Guildford & Waverley areas.

We provide a wide range of adult community health services in north west Surrey, and children's NHS community services across the whole of Surrey. The children's service - Children and Family Health Surrey - is delivered through a partnership service comprising CSH, Surrey's mental health provider Surrey and Borders Partnership NHS Foundation Trust, and another community provider First Community Health and Care.

**What you’ll be responsible for**:

- The post holder will have responsibility to manage and deliver care for families, children and communities working in partnership with team members and service users.
- The role will provide a proactive, responsive service based on identified health needs whilst leading the delivery of the universal Healthy Child Programme. He/she will ensure effective delegation, work load management and monitoring of care standards.
- The practitioner will work collaboratively with other health care professionals and agencies ensuring a seamless delivery of services for children and families within the community
- Assess child and family needs, using the appropriate assessment and risk assessment tools. Ensure that at each contact with a child an assessment of the health and developmental need is conducted.
- Develop and implement outcome focused care plans and evaluate care given, in partnership with clients.
- To provide support to families using the Solihull approach model in behaviour management, parenting skills and delivering programmes of care to vulnerable families.

The role will involve, but not be limited to you performing the above.

**You’ll have the following qualification/experience**:

- A minimum of 6 months paid experience in a Health Visitor role within the last 3 years.
- Comply with NMC Professional Registration and keep up to date with current Health Visitor practices.
