Job Description: Care Coordinator
Position Overview:
The Care Coordinator plays a vital role in ensuring the smooth coordination and delivery of care services to clients. They are responsible for managing and organizing the schedules of care staff, coordinating client care plans, and maintaining effective communication with clients, their families, and the care team. The Care Coordinator works closely with other members of the management team to ensure high-quality care is provided in accordance with company policies and standards.
**Responsibilities**:
1. Schedule and Coordinate Care Services:
- Develop and maintain care schedules for clients, ensuring appropriate coverage and continuity of care.
- Assign and allocate care staff based on client needs, preferences, and availability.
- Monitor and manage daily schedules, making adjustments as necessary to accommodate changes or emergencies.
2. Client Care Coordination:
- Collaborate with clients and their families to develop personalized care plans that meet their specific needs and preferences.
- Conduct regular assessments to evaluate and update client care plans as necessary.
- Coordinate and oversee the delivery of care services, ensuring they are provided in accordance with the care plans and quality standards.
3. Staff Management:
- Recruit, interview, and select care staff, ensuring they meet the required qualifications and credentials.
- Provide orientation, training, and ongoing supervision to care staff to ensure they are equipped with the necessary skills and knowledge.
- Conduct performance evaluations and provide feedback to care staff, addressing any areas of improvement or recognition.
4. Communication and Collaboration:
- Maintain open and effective communication with clients, their families, and the care team to ensure all parties are informed and involved in the care process.
- Collaborate with healthcare professionals, community resources, and external agencies to coordinate additional services or support for clients as needed.
- Act as a liaison between clients, their families, and the organization, addressing any concerns or issues promptly and professionally.
5. Documentation and Compliance:
- Ensure accurate and timely documentation of client care plans, schedules, and any relevant information.
- Adhere to all regulatory requirements and company policies, ensuring compliance with standards of care and best practices.
- Participate in quality improvement initiatives, identifying areas for enhancement and implementing necessary changes.
Qualifications:
- Bachelor's degree in a related field (Social Work, Nursing, Healthcare Management, etc.) or equivalent experience.
- Previous experience in a care coordination or similar role within the healthcare or social services sector.
- Knowledge of care planning, scheduling, and coordination principles.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Excellent communication and interpersonal skills, with the ability to build rapport and effectively collaborate with clients, families, and the care team.
- Understanding of relevant regulations, policies, and standards related to care coordination and services.
**Salary**: £20,032.36-£36,878.03 per year
