SRS Care Solutions Central are looking for a **Home Care Coordinator** who shares our values of; **compassion, respect, wellbeing, dignity and inclusivity**
As a **Home Care Coordinator**, you are responsible for managing the staff and client rota’s by completing each in advance, using the **Birdie** scheduling system. You will also be responsible for updating client files use of the internal IT system. This is an office based position working **35 hours** a week.
- To be aware of Service Users needs using person centred outcome planning.
- Work in partnership with the Service/Registered Manager and Service Users in order to successfully match staff that will meet the individual needs of the Service User.
- Maintain detailed person-centred outcome focused support plans, risk assessments and Service Users’ files, folders and all relevant documents.
- You will be responsible for coordinating the shift cover for Home Care Support Workers, which is some instances may include you providing cover.
- You will be required to participate in an out of hours rota.
- Ensure that all changes to service users schedules are reported without delay and arrange replacement staff.
- You will be responsible for updating support plan, updating service users’ files and other administrative tasks as and when required.
- Provide high quality person centred personal care to service users including intimate procedures as required.
- Attend any relevant meetings as instructed by the Service/Registered Manager.
- Participate in staff induction as and when required.
- Ensure that all service users and staff files are up to date in accordance with company policy.
You will attend all relevant training to ensure you are at a standard set by Health and Social Care Standards and SSSC Codes of Practice and have the responsibility to keep your SSSC registration up to date.
S/NVQ level 3 in Adult Health and Social care or willingness to work towards.
**Job Types**: Full-time, Permanent
Pay: £25,000.00 per year
