**Job Details**

The role will include keeping accurate records of outcomes focused support planning and review for the people whom you support, ensuring changes in service user health and wellbeing, support plans, risk assessments etc. are recorded and communicated across the team.

You will work as part of a team liaising with colleagues to ensure an integrated, multi-disciplinary approach to service delivery.

Service users may expect their keyworker to plan with them ways of meeting their individual needs, keeping accurate records of outcomes focused support planning and review for the people whom you support.

In return, you will be part of a team who can provide 24 hours support to people in their own homes both single tenancies, group homes and core and cluster services. accordance with the National Care Standards shift work.

**The Person**

If you are caring, compassionate, want to make a difference in people lives that you support, have a sense of fun, and enjoy working with people with disabilities, mental health. Please apply.

Must have or be willing to achieve in the first period of registration, the Social Services and Healthcare SCQF level 6/7 (equivalent to SVQ3) or equivalent to meet Scottish Social Services Council registration.

Duties include all aspects of personal care and have the ability to undertake all moving and handling tasks including the pushing and pulling of wheelchairs.

You are required to register with Scottish Social Services Council (SSSC) within 6 months of taking up employment in this role and to complete the required Post Registration Training and Learning (PRTL) over the first registration period.

**Before confirming appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme **.

**You will save money on your SSSC fees as these will now be paid for all roles and grades by the Scottish Government.**
