The successful Care Coordinator must be able to communicate professionally with clients and care staff to ensure the smooth running of the contract.

**The Care Coordinators main duties include**:

- The organisation and coordination of care services in accordance with current best practices, according to policy and procedures, agreed-on standards, legislative requirements, and relevant regulation under the direction of the Registered Manager.
- To organise, coordinate and supervise care services within the agency in accordance with agreed standards.
- Systematically solve day-to-day problematical issues which arise.
- Ensure that care services are delivered to clients at the times and for the duration specified.
- Ensure that the staff allocated, and the staff attending, to each client has the relevant skills, knowledge, and resources related to the needs of the client.

- Rota planning for staff: (required)
- domiciliary care (required)
- Driving Licence (required)
- Care coordinator experience in a domiciliary care setting (essential)
- Level 3 NVQ in health and social care and working towards level 4 (essential)
- ICT knowledge (Desirable)
- Understanding of the 5 Key Lines of Enquiry (Desirable)

- Monday to Friday
