Do you have experience in Health and Wellbeing Coaching? Are you looking for a job where you can make a difference? Torridge Primary Care Network is a group of practices working together to improve patient care and outcomes, located in beautiful North Devon. We are excited to be recruiting two Health and Wellbeing Coaches to work as part of a multi-disciplinary team within the PCN. This is a 12 month maternity cover fixed contract role, you will work in partnership with both clinical and non
- clinical colleagues, managers and the wider PCN to ensure the role delivers the best possible outcomes for our patients and to ensure we have an inclusive team environment.

The Health and Wellbeing Coaches will deliver, both group and one to one sessions, on healthy eating, physical activity and health and wellbeing to meet with personalised care plans. You will play a critical role in engaging patients and use health coaching techniques to support them to take an active role in their health and wellbeing.

The post holder will have a key role in helping to raise the local population’s awareness of the support groups and opportunities available to assist them in achieving their health and wellbeing goals.

We can offer a fantastic and supportive working environment and the opportunity to grow and develop with the PCN, along with 5 weeks annual leave plus pro-rata bank holidays and an NHS pension. The salary is based on Band 5 pay scale.
