**Salary**: Band 2 £23,362 - £25,368 per annum, pro rata

We are looking for individuals who have a passion for hospice care and would like to take the hospice model of care into people’s own homes at the end of life.
- Have experience of working in a Hospice In-patient Unit or equivalent expertise in care of the dying.
- Be experienced in providing practical, emotional, social and psychological support to individuals at the end of life as well as supporting their families and carers
- Be registered with SSSC or achieve registration within first 6 months of role.
- Be qualified or willing to study towards achieving the qualification required by the SSSC for ‘workers in a care at home’ service (HNC Social Services or SVQ2 in Social Services and Healthcare)
- Have the ability to be flexible in order to meet the needs of the service - which will be provided over seven days and day/night.
- Be comfortable working on your own and as part of a wider professional team
- Have excellent communication skills
- Have experience in working in community settings
- Be able to work in isolation
- Be able to travel independently to provide care across the community. (Full UK Driving License is essential).
