The post holder, as a member of the Community Nursing Service, will visit patients in the community setting to undertake venepuncture as requested by other health care professionals. The post holder will deliver this service without direct supervision of a qualified nurse, but under the clinical leadership of the senior staff nurse. The post holder will be expected to participate in the training of individuals including qualified staff.

**The main tasks you will spend your time doing are**:

- Clinical Direct, for instance home visits
- Clinical Indirect, for instance patient documentation, travel to and from their home visit, and liaison with other professionals.
- Non-individual Patient Attributable, for instance MDT meetings, handover and safety huddles, clinical triage, allocation, caseload management, and prescription and equipment ordering.
- Supporting Professional Activity, for instance imparting clinical knowledge, mentoring, giving and receiving training, and staff and team development.

1. To collect venepuncture supplies / equipment
2. To prioritise workload
3. To provide advice, support, guidance and information to individuals where appropriate, escalating concerns to the General Practitioner, Registered Nurse or Registered therapist as appropriate
4. Maintains accurate and contemporaneous
5. Delivery of interventions delegated by registrant i.e. Undertake observations and escalate as appropriate, provision of personal care and support, installation of patient specific eye and ear drops, INR near patient testing and assistance with double handed visits.
6. Inputs and records activity and outcomes to meet contractual
7. Participates in Trust audit programmes
8. Participates in the Trust Supervision
9. Participates in induction and orientation programmes of new
10. Participates in training and education programmes for staff, students and others as required.
11. Contributes to the efficient working of the Community team, including specific clerical duties and delivery, cleaning and inventory of medical equipment / devices/ dressings as appropriate.
12. Participates in clinic sessions, as appropriate
