At Connection Support, our team of staff members and volunteers work alongside people to overcome life’s challenges, with the goals of solving homelessness and achieving independence. We do this through our values of empowerment, collaboration, compassion and taking a personalised approach in all aspects of our work.

We are currently recruiting an Embedded Mental Health Worker to join our small, friendly team.

**About the role**

Working mainly at The Warneford hospital, and across Oxford University Hospitals, Oxford Health Foundation Trust sites, and as part of a wider team led by Oxford City Council, you will support individuals experiencing a crisis or deterioration in their mental health to resolve any difficulties they may be having with their accommodation.

Using a strength-based approach, and assertive outreach, you will support people to increase their confidence, develop routines, build skills and plan for their future. This may include accessing support for accommodation, debt, physical and mental health treatments, substance misuse, engaging in activities, building networks, and maintaining contact with friends and family.

In addition, you will review cases and processes to see if additional support could have been put in place earlier, to reduce instances from occurring and to allow for an increased effectiveness of inter-agency work.

This post is fixed term until 31st August 2024, working 22.5 hours per week, Wednesday - Friday.
- **Starting salary £25,380 - £28,949 per annum, pro rata, dependent on experience.**_

Due to the nature of the role, you will need a current drivers licence and access to a vehicle.

**About you**

You should have significant demonstrable experience and confidence in supporting people who are experiencing mental health issues. In addition, you will understand and have used strength-based support and have a good level of housing and resettlement knowledge.

You have the ability and patience to support people in providing information and understanding how to access and navigate support services. You also understand the importance of researching information about local resources and activities.

Your natural ability to connect with people, and your excellent listening skills allow you to approach difficult situations with a patient, calm approach, and you understand the importance of maintaining professional relationships with hospital staff and other agencies.

Ideally, you will have used Microsoft Office and Outlook and have some experience of recording and maintaining information. You have a strong resilience to not give up and can work creatively to find solutions.
