These posts are funded through the additional role reimbursement scheme (ARRS) and provide an exciting opportunity to be part of the Frodsham, Helsby and Elton Primary care Network (PCN).

Working in partnership, the role will be directly employed and supported by CWP, but fully embedded within the Multi-disciplinary PCN team to develop new ways of working and provide the essential connections between primary and secondary care services.

Work already undertaken has laid the foundations for this new and exciting way of working, and a support network of other Primary care mental health practitioners has been established.

The role of the Primary Care Mental Health Practitioner will be to work within Primary Care and Mental Health services, facilitating mental health assessment in primary care.

The post holder will be working with clinical teams to deliver the best primary care healthcare service to patients, linking in and liaising with local community services, other NHS departments and the wider health and social care networks.

The role will include providing prompt local assessment, triage, risk assessment, risk management, signposting, onward referral, and evidence-based interventions for patients presenting with mental health problems.
The role will operate within the principles of recovery and be responsible for the case management of agreed individuals with mental health conditions in conjunction with the Primary Care Team, liaising with GP’s and other allied healthcare professionals to manage patients with a wide range of presenting mental health conditions.
