We have an exciting vacancy for Community Advanced Nurse Practitioners (CANP)
The Community Advanced Nurse Practitioner will understand the constructs of community nursing teams and would be willing to further develop into an ANP role. The practitioner will work autonomously as an integral part of a community nursing team in North Lanarkshire HSCP across seven days to meet the demands of the service.
The CANP will work within the scope of their professional practice, demonstrating an awareness of their own limitations in knowledge, understanding and clinical competence, and recognize when to seek expert advice. The CANP will work closely with the Community Nursing team, alongside the wider multi-disciplinary team and other health care professionals. The CANP will communicate where relevant with GPs about any patient they have seen with their referrals and work being generated from the Community Nursing team.
The CANP will be responsible for the clinical decision making, assessment and treatment of patients which have been escalated to them from the community nursing team caseloads. The post holder will be a clinical expert practitioner and will manage all aspects of the patients’ care.
CANP will provide assessment, clinical care and management of both stable and acutely unwell patients making complex, autonomous decisions using advanced skills and knowledge, with overall responsibility for the assessment of care needs and the delivery of advanced nursing care within the CANP caseload.
Additional Information Specific to the Role:
- In order to be considered for the post you will need to hold a Post Graduate Diploma in Advanced Practice with community experience as a caseload holder?
- Annex 21 will be considered for anyone who is not a qualified Advanced Nurse Practitioner but who is willing to undertake Post graduate diploma in advanced practice and undertake competency completion and sign off as an advanced practitioner. The CNAP will work within the NHS Lanarkshire Advanced practice governance framework.
The post holder will possess excellent team-working/leadership skills and have the ability to motivate and influence others and work using own initiative as well as excellent listening, communication and interpersonal skills with effective time management skills.
Working knowledge of basic information technology.
