**Community Engagement Coordinator - Job Description**

**Reporting to**: PCN Operations Manager/PCN Manager

**Hours**: 37.5 hours per week

**Salary**: £11.77 per hour

**Base**: Based at Kingswood Health Centre, but travel within the PCN area is required

**Job Summary**

4PCN’s vision is to de-medicalise health problems for its patients as much as possible, and enable patients to live a healthy, happy life in the local community. The Community Engagement Coordinator role will help to coordinate, develop and promote community initiatives, services and schemes that will benefit the health and wellbeing of our patients, paying particular attention to tackling health inequalities and improving the patient experience of local healthcare.

**Major Duties and Responsibilities**

**External**
- To act as an ambassador for the PCN and its constituent practices to promote and grow our involvement in community initiatives.
- To build a knowledge of local community organisations, schemes and services that can benefit patients and their health, both directly and indirectly.
- To contribute to local forums, services and schemes the perspective of the local GP surgery, and how we can work together to improve outcomes for the community.
- To maintain awareness of local and national initiatives involving health and social care that might benefit patients.
- To investigate and help source funding to run PCN-based services/groups.

**Internal**
- To work with clinicians, staff and third parties to investigate opportunities and propose ideas for new schemes and services that might benefit our patients.
- To establish support groups or host educational events for specific cohorts of patient and their carers/relatives.
- To help manage volunteers who support schemes and initiatives undertaken.
- To help inform PCN practice policies and pathways as to how we can benefit from or with the community or third-party initiatives.
- To establish and run the PCN Patient Participation Group, liaising with the existing practice PPGs - chairing meetings, creating agendas and coordinating volunteer activities.
- To work with practices and patients to enhance the patient experience when contacting the surgery - seeking feedback from service users and working with practices to act where improvements can be made.
- To promote and educate patients on the variety of NHS services available to them and to access the “Right Care, First Time”.
- To promote and educate patients on the benefits and use of online systems within their practice and the NHS app.
- Be responsible for PCN updates on practices websites and social media, promoting schemes and initiatives being undertaken by the PCN team.

**Supervision Received**

The postholder will be operationally line managed by the PCN Operations Manager, with feedback and support received from the PCN Manager, Clinical Director and other members of the PCN board.

**Supervision Exercised**

The postholder will not have any operational line management responsibility.

A good standard of general education, educated to GCSE Level

A qualification in Health and Social Care, or a related discipline

A commitment to continuing personal and professional development

Skills/Experience

Experience of working with the public

Experience of working in the NHS, and ideally General Practice, with an understanding of the current pressures faced

Experience of engaging with multiple organisations and/or stakeholders

Experience of delivering projects across public or third sector organisations

Strong interpersonal skills and the ability to communicate effectively with staff, patients, and stakeholders alike

Experience of working with the voluntary sector

Able to prioritise workload effectively both individually and within a team

Fully IT literate including Microsoft Office
